I hereby give permission to the Chris Wormley Summer Youth Football Camp to administer any medical attention that is deemed necessary to my child in the event that injury occurs while he or she is participating in the camp. I also acknowledge the Summer Camp is not responsible for any injuries suffered at, in route to, or from the camp. I give permission to the Summer Camp to take photographs of my child during the camp.
